LHS PTSA Student Directory Information

Each year, the LHS PTSA publishes a directory of student names, phone numbers, and addresses. The Student Directory also includes phone numbers and email addresses for all faculty and staff. This directory is for the personal use of PTSA members only. It is not to be used for commercial purposes.

Families must give permission to publish student information by signing the Student Directory Form and returning it to the PTSA before the indicated deadline. The PTSA is required to obtain parental permission for each child entering LHS, even if permission has already been given for older siblings. Once permission is given for a child, it is in effect for the duration of the time that child is at LHS. You need not join the PTSA to have your student’s information published in the directory. However, to receive a copy of the student directory, membership is required.

Please print the directory/membership form and mail it with your check, payable to LHS/PTSA, to the appropriate person according to the directions at the bottom of the form.
