LHS H1N1 Clinic

On December 2nd, the Town of Lexington Health Department will hold an H1N1 vaccine clinic at Lexington High School. LHS students will have the opportunity to be vaccinated during X-block, lunch block, and after school. LHS students will be receiving consent form packets during homeroom November 17th. Please have your LHS student return the completed consent form as soon as possible to their house secretary no later than the following Tuesday November 24th. Consent forms may also be returned on the 24th to their homeroom teacher. LHS students may not be vaccinated without a consent form signed by their parent/guardian.

ENTRY PHYSICALS
All Kindergarten, 6th grade and 9th grade and students new to Lexington need to submit a physical examination form to their receiving school’s nurse prior to entry into school. Please also request your pediatrician do a tuberculosis risk assessment at the time of the physical.
IMMUNIZATION INFORMATION
MEASLES BOOSTER
A measles booster is required to enter Grades K-12
TETANUS BOOSTER (TD)
A tetanus booster is required for entry to Grades 7 – 10 if last tetanus immunization was 5 years or more.
CHICKEN POX
Verification by MD of disease or Varicella vaccine given per DPH age requirements for entry to Grades K-3 and 7-10. One dose prior to age 13. Two doses if age 13 or older.
HEPATITIS B SERIES
A series of 3 Hepatitis B immunizations is required for entry into K-10.
If you feel you have a financial need or no insurance, please contact your school nurse or you may wish to contact the Children’s Medical Security Plan (1-800-909-2677), an insurance program sponsored by the Massachusetts Department of Public Health.
MEDICATION POLICY
In compliance with Massachusetts General Law and for the safety of our students, this medication policy has been written and will be strictly enforced.
The policy for administration of medications, whether prescribed or over-the-counter, during school hours, is as follows:
Medication must be accompanied by a medication permission form signed by both the physician and parent. For short term medications such as antibiotics, the prescription bottle is acceptable for a physician’s order and the medication permission form or a written statement by the parent is also required.
Medication must be supplied by the parent in the original pharmacy container. (Please ask your pharmacist to provide a second container and send only the amount of medication needed to school.)
Medication is kept locked in the nurse’s office and is dispensed by the school nurse. For your child’s safety, and the safety of other students, students are not allowed to carry medication at school. When a physician deems it necessary for a student to have immediate access to medication (inhaler), the parent will provide documentation from the physician stipulating such necessity and confirmation that the student has been advised of cautions and proper use of the inhaler in school.
All medication orders must be for treatment of a specifically diagnosed medical need and must be renewed at the beginning of each school year.
The parent may retrieve the medicine from school at any time and the medicine will be destroyed if it is not picked up within one week following termination of the order or one week beyond the close of the school year.
